General Information and Intended Use

This application form has been prepared by Tolunay Akay Attorneyship & Consultancy (“Tolunay Akay”) in accordance with the “Communiqué on the Procedures and Principles of Application to the Data Controller” for your use within the scope of “Application to the Data Controller” within the scope of Article 13 of the Personal Data Protection Law No. 6698.

The applicant’s request is answered as soon as possible and in any case within 30 days at the latest, depending on the nature of the request. We would like to remind you that if you are not satisfied with the answer we have given you or if we reject your application, you can apply to the Personal Data Protection Board within 30 days. In order to exercise this right, please make sure that you first apply to Tolunay Akay Attorney & Consultancy, which is the data controller in accordance with the relevant law.

Application Procedure

This form, prepared in accordance with the Communiqué on the Procedures and Principles of Application to the Data Controller, is provided for your convenience to ensure that you make an application in accordance with the mandatory elements set out in Article 5 of the Communiqué. If you wish, you may submit this form or any other written text. In addition, you may submit your requests by any method determined by the Personal Data Protection Board and in person, by mail, electronic mail, or cargo to the address below.
